Band 6 Perinatal practitioner for Perinatal Parent Infant Mental Health Service
ADDRESS: Ilford, Essex
WORKING HOURS: 5 sessions over 2.5 days – negotiable working days (ideally one day should be a Tuesday but not essential)
DURATION: Minimum 3 Months
RESPONSIBILITIES:
• To manage a caseload of service users open to the perinatal service, offering a range of interventions, for a stated limited period
• Ensuring assessment, support/care-plans and intervention is shared and agreed with relevant individuals and agencies
• To plan care in association with other members of the multi-disciplinary team, involving involve service user, and where appropriate carers in the care planning and care delivery, having explained options and obtained consent to care and treatment
• To be responsible for organising and prioritising workload in the day to day allocation of work, responding to referrals as and when they present
• To have organisational knowledge relating to Trust protocols and procedures and adhere to them
• To be responsible for providing accurate records of information required by the Trust for audit purposes
