Job Description
• Monday – Friday 9am-5pm
• Care Coordinate a caseload of patients with severe and enduring mental illnesses
• Complete risk assessment and client- centred care plans
• Provide face to face treatment in the community
Position Requirements
• NMC registration
• Previous clinical experience performing a similar role in the NHS.
• Enhanced DBS
