Band 6 Senior Mental Health Practitioner – Essex
IMC Locums are looking for a Band 6 Community Mental Health Practitioner to work in our reputable NHS Trusts based in North East London.
Job Description
• To work as part of the EIP team, day to day responsibilities in providing holistic care approach centred to individuals needs within the FEP pathway as recommended.
• Conducting initial assessments of new referrals, identifying and signposting to relevant services, care coordination and caseload management,
• Participate in MDT, ward reviews and other professionals meetings as when necessary,
• Ensuing that all patients PH are up to date, administration of medications, depot etc
• Risk management and care plans as well as duty work on a rota basis.
• ASAP start date – Ongoing
• Mon – Fri (37.5 hours)
• Rates up to £27p/hr
Position Requirements
• Valid NMC registration
• Enhanced DBS
• PMVA training
• Driver’s license
