IMC Locums are looking for a Band 6 Senior Mental Health Practitioner to work within a reputable NHS client based in Swindon.
Job Description
•The Older Adults Community Mental Health Team are looking for a Band 6 Care Coordinator to cover the Henley area.
•Working mainly out of the Wallingford office with the occasional day in Abingdon where the main team is based, this is a contract of at least 3 months initially and likely to be extended.
Applications are welcomed from RMNS, Social Workers and OTs who have mental health community experience, we are looking for a full time person but part time applicants will also be considered.
•Hours of work are Monday-Friday 9-5 (could be 4 longer days)
•Minimal WFH as this is mainly a face to face role.
Position Requirements
•Care of Older adults with mental health needs both functional and dementia
•Assessment of mental health needs, care planning with psychosocial interventions and medication.
•Care Coordination.
•Valid NMC Pin
