Job Description

The Urgent Crisis Response Team (CRT) is a multi-disciplinary team that supports patients at home either when in a crisis to prevent an ED attendance, thus a hospital admission or to support early discharge allowing patients to get the care they need at home safely and conveniently, rather than being in hospital.

The service has the ability to clinically monitor patients remotely using enhanced technology and a highly skilled clinical team that can respond within 2 hours.

The team comprises of Advanced Clinical Practitioners, Senior Nurses Occupational Therapists, Physiotherapists, Social Workers, a Pharmacist and Senior Support Workers.

The role of the Senior Therapist within the service is to work collaboratively with other team members offering holistic assessments for patients who may be in a crisis situation. There is a strong element of “see and treat” management within the therapy role in the community setting.

 

 

Position Requirements

  • HCPC Registered
  • 6 month (minimum) locum assignment to start as soon as possible
  • Full time hours (Monday – Friday)
  • The successful candidate will be working within a strong team.
  • Minimum of four years postgraduate experience
  • Previous clinical experience performing a similar role within a Community setting
