Locum Band 6 Intermediate Care Physiotherapist – Essex

We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in Essex. They are seeking a qualified Therapist with previous Intermediate experience.
Job Description
- Provide expert knowledge and skills to support the multi-disciplinary team to facilitate effective prevention of admission to hospital or residential care and facilitation of discharge from hospital to the Intermediate Care Services
- Manage and co-ordinate a caseload of patients with complex conditions who are undergoing a rehabilitation programme
- To oversee physiotherapy provision and ensure that accurate records and risk assessment are maintained and regularly reviewed
- To provide expert leadership to junior staff through support, supervision and appraisal
- Work in close liaison with all members of the local MDT including Reablement, Social Services, Hospital and Discharge Team to achieve and maintain smooth care pathways for patients
- Work in conjunction with the SPOR Matron and Intermediate Care Lead on strategic development of IC services in line with future service changes as necessary
- Assist in the collection of information to support the evaluation of services once commissioned.
Position Requirements
- HCPC Registered
- Minimum of two years postgraduate experience
- Previous clinical experience performing a similar role within a Intermediate Care setting.
