Locum Band 6 Discharge Occupational Therapist – Birmingham

We have a fantastic opportunity for a locum Occupational Therapist to join a leading teaching hospital, based in Birmingham. They are seeking a qualified therapist with previous Discharge experience.
Job Description
- assess new patient admissions, where possible jointly with physio
- identify short term goals, achievable within 2 weeks.
- devise a treatment and discharge plan with physio in accordance with Discharge 2 Assess model.
- liaise with patients and relatives about discharge plans.
- attend daily MDT tracker meetings
- complete necessary access visit assessments
- prescribe and order equipment
- refer to community therapy- Early intervention community team
- devise plan care plan for requesting package of care
- refer to social worker if required
- mental capacity assessment
- liaising with out of area local authority and ordering equipment and community therapy.
- Support Band 5 with caseload and report to Band 7 with any complex cases.
Position Requirements
- HCPC Registered
- Minimum of two years postgraduate experience
- Previous clinical experience performing a similar role within a NHS Hospital.
