Locum Band 6 Community Physiotherapist – Essex

We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in Essex. They are seeking a qualified Physio with previous Community or rehab (inpatient) experience needed.
Job Description
We are looking for a Band 6 Physiotherapist to work in a Community setting, for either 30hrs or 37.5hrs each week Mon-Fri can be flexible hours each day between 8am-5pm.
Position Requirements
- To deliver high quality, skilled clinical care to patients at home and provide rehabilitation and long term care/conditions management in a variety of community settings.
- To manage a caseload of patients with complex health & social care needs, using evidence based, client-centred principles to assess, plan, implement & evaluate interventions
- To utilise specialist skills and knowledge to holistically assess needs and provide programmes of rehabilitation based on evidence based practice.
- To work, as required, on a flexible basis to cover all roles required within the team
- To work within and across professional and organisational boundaries, liaising with GP’s other professionals and statutory and voluntary agencies to deliver individualised and expert clinical care to a high standard to clients.
- To take responsibility for managing the caseload and the team, in the absence of the band 7
- To support the Locality Manager/Clinical Lead and colleagues in delivering high quality care that delivers best value, and meets the requirements of commissioners.
- To be personally responsible for all aspects of safety relating to equipment used when treating the patient or issued to the patient for their own use. To be responsible for checking and/or teaching competency of non-qualified staff in using equipment e.g. walking aids, appliances etc.
- To develop collaborative treatment programmes with patients, carers and colleagues, demonstrating client centred reasoning and specialist problem solving techniques.
- Evaluate patient progress, reassess and review treatment programmes as required through the monitoring of medical and rehabilitation needs and adjusting as required. This will involve determining the potential for rehabilitation and future care and support needs
- Plan and prioritise own workload and delegate tasks where appropriate to ensure effective use of time.
- To produce timely and accurate electronic patient’s records and reports in accordance with Health Care Professions Council and Chartered Society of Physiotherapy regulations..
- Identify and undertake routine and complex risk assessments, both of environmental and individual case management. Advise the multi-professional team in determining appropriate management relating to moving and handling and safe use of equipment.
