We have a fantastic opportunity for either a locum Physiotherapist or Occupational Therapist to join a leading teaching hospital, based in Lancashire. They are seeking a qualified therapist with previous NHS experience.

Job Description

JOB SUMMARY:

The post holder will be working within a service, where tasks and duties will evolve as the service further develops. It is important the post holder is able to work flexibly, be adaptable to change and at times use their own initiative to complete tasks.

Work within and adhere to the Royal College of Occupational Therapists (RCOT) Code of Ethics, Chartered Society of Physiotherapists (CSP) Code of Ethics and Professional Conduct, The Health professionals Council (HPC) standards of practice, The Nursing and Midwifery council (NMC), The Trust relevant policies and guidelines and also National guidelines.

The post holder will be an autonomous practitioner, responsible for provision of triage to facilitate discharge planning, assessment, treatment, intervention of clients referred to the service.

The Band 6 post holder, as Clinical Co–ordinator, will work within 2 key areas:

Clinical Triage- working alongside local authority staff and health colleagues to gather information from available sources/computer systems. This joint triage will decide the relevant onward referral pathway for patients to facilitate their discharge from the acute setting and Clifton Hospital.

-To recognise the need for intervention/advice from a Senior Clinical Co-ordinator.

Home first- to facilitate discharge from the acute hospital and Clifton hospital to the patient’s own home completing an assessment of ongoing needs and following the key home first principles:

Position Requirements

  • HCPC Registered
  • Minimum of two years postgraduate experience
  • Previous clinical NHS experience
