Job Requirements:
This position is for a community rehab post. Essentially working in the community in a patients home, this also includes care homes. The role works closely with the acute hospitals to support a timely discharge and also aims to prevent admission. The majority of patients are elderly and have comorbidities that include MSK, neuro, orthopaedic and medical needs.
