We have a fantastic opportunity for a locum Physiotherapist to join a leading teaching hospital, based in Cheshire. They are seeking a qualified Physiotherapist with previous Community experience.
Job Description
To be responsible for providing Specialist assessment and treatment to patients with diverse health needs in a variety of community rehabilitation set up. Ensuring appropriate pathways of care and communications via liaison and referral to other agencies as required.
SERVICE DELIVERY
1.To contribute to the effective day to day provision of Therapy in your working area, reporting to and liaising with the team leader and manager.
2.To ensure patient receives highest quality of therapy and to facilitate timely discharge from the Community hub.
3.To work in a generalist specialist role in a lone setting, without the benefit of detailed patient records. Assessing and delivering treatment to clients presenting with complex and diverse health needs in a variety of environments.
4.To manage own complex case load of referred patients. Monitoring the appropriateness of referrals, obtaining information relevant to patient care and ensuring appropriate discharge/ongoing care.
5.To maintain accurate professional documentation in line with the HCPC standards and local policies and procedures. Contributing to documentation monitoring as required.
6.To develop effective systems of communication and liaison with referring agents and all members of the multi-disciplinary team. Working across boundaries with partner and voluntary organisations. Providing and receiving highly complex information, sometimes of a sensitive nature, using developed interpersonal and communication skills where co-operation is required and there may be barriers to communication.
Position Requirements
•HCPC Registered
•Minimum of two years postgraduate experience
•Previous clinical experience performing a similar role within a Physiotherapy setting
