Job Title: Band 4 Senior Community Therapy Assistant

Location: 2^nd Floor, The Old Forge, 45-47 Peach Street, Wokingham, RG40 1XJ

Start Date: ASAP

End Date: 30/07/2022

Working Pattern: 37.5 flexible

Shift Pattern: 8:00-16:00 or 14:00-22:00 (PT or remote working)

Specific Skills:

To undertake initial assessment and ensure patients are taken on through the appropriate service and have a clear care plan
To discuss patients progress and review/liaise with the multi-disciplinary team
To holistically support the patient throughout the re-ablement process and provide palliative care
To act as allocation co-ordinator and help with shift
To pursue the most appropriate action in a timely manner on behalf of the referrer or patient.
Fully UK license and access to a car (E)
Previous experience in care and therapy roles (E)
Fully registered Enhanced DBS (E)
